Supportive Housing to Help You Live Independently and Thrive

Yakima Housing Authority proudly offers Supportive Housing Services through Washington State’s Foundational Community Supports (FCS) Medicaid Transformation initiative.

Supportive Housing Services help eligible Medicaid clients find and maintain safe, stable housing. Through this service, Yakima Housing Authority (YHA) provides long-term, person-centered support that promotes independence and community integration.

This service is part of the Foundational Community Supports (FCS) program — a Washington State Medicaid initiative that supports people with complex needs who are at risk of homelessness or institutionalization.

Our Supportive Housing Services Include:

We help individuals identify their housing goals, preferences, and community integration needs through a collaborative assessment process.

We guide clients to local housing opportunities and community supports that match their individual needs — ensuring a smooth transition to permanent housing.

Our team educates clients on the responsibilities of being a tenant and supports them in understanding lease agreements and tenant rights.

We help individuals develop essential daily living skills such as budgeting, grocery shopping, household management, and planning for long-term success.

YHA works with a network of trusted landlords. We foster positive communication between clients and property managers to help resolve concerns and build housing stability.

If a housing crisis arises, our staff steps in to advocate for individuals and mediate issues — helping prevent eviction and ensuring continued housing access.
